in June 2005
Refunds: If you pay and then notify Mike Bryan that the
scout must cancel, we will refund everything that Camp Goshen returns
to us on your behalf. All refunds are subject to a penalty from Camp Goshen.
Camp Goshen will not provide refunds on cancellations within two weeks
of camp except for medical reasons with a doctor’s note. You will
not receive a refund of any amounts that the Pack has already spent on
the scout’s behalf by the time of the cancellation, e.g. the cost
of camp T-shirts.
Medical forms: Scouts need a BSA Class 1 / Class 2 medical
form for summer camp. The Class 2 portion of the form, and the medical
exam that it describes, are good for 3 years. Consequently, if you have
a Class 1 or 2 form from last year or the year before, a parent should
make any necessary pen-and-ink changes to the Class 1 portion (e.g., scout’s
age) and then re-sign and re-date the Class 1 portion at the bottom of
page 1. Blank Class 1 or 2 medical forms are available here.
Goshen will not return them to us (the forms, not the scouts), so to save
a physical exam next year, make a copy of the form before submitting it.
Medical forms are due by June 2005. They may be turned in any time before
that; the sooner the better. Please start looking for last year’s
form soon. If you can’t find a form reflecting an exam that has
taken place since August 7, 2002, the scout will need an exam.
Adults 40 or over attending summer camp require a Class 3 medical form,
available here. A Class 3 physical exam is only
good for one year.
If your son will be taking medication at camp, give the medication and
written instructions to the trip leader on departure day. Instructions
should include dosage, frequency, and conditions such as before or after
meals. If your son is allergic to any medication, note this under "Allergies"
in the Class 1 portion of the medication form.
Permission slip: See here;
please return this with your check. The slip includes a statement of understanding
on payment and paperwork deadlines and policies.
If you have any questions, please let me know.
